Care Plus

What is Care Plus?
Care Plus is available through a Primary Health Organisation (PHO) like Harbour Health.   The aim of the programme is to improve patients quality of life by working closely with the patient to:

  • improve the health professionals understanding of their health issues/concerns,
  • work closely with them to improve their knowledge of their condition,
  • work with the patient to identify and set achieveable lifestyle changes.

Who is eligible for Care Plus?
Your family doctor or nurse can make an assessment of you to see if you are eligible for Care Plus. It is aimed at people who need to visit their family doctor or nurse often because of significant chronic illnesses, a terminal illness, two acute non-surgical admissions in the past 12 months or is on active review for elective services.

How do I enrol for Care Plus?
First talk to your usual family doctor or nurse to find out if they are offering Care Plus. Currently not all family doctors are offering the service. Look for your nearest Care Plus affiliated Harbour Health Doctor.

What difference will I notice with Care Plus?
An initial comprehensive assessment will be followed by regular assessments. An individual care plan will be developed with you setting, achievable health and lifestyle goals. As a patient on Care Plus you will receive support from your family doctor and nurse who will assist you to achieve effective management, a better understanding of your conditions and support to make lifestyle changes.

How much will Care Plus cost me?
These services will be provided at a low or reduced cost.

Can people who are not enrolled with a PHO family doctor get this new service?
This service is only available for people who are enrolled in a PHO. Contact your local family doctor or view our affiliated Medical Centres to enrol with one nearest to you.

Is Care Plus replacing the High User Health Card?
Over time, Care Plus will replace the High User Health Card (HUHC). Care Plus is different from the HUHC because it provides a comprehensive approach to improve the health and well being of people with chronic conditions, by focusing on making healthy and improving lifestyle choices. 

Will High User Health Card patients automatically transfer to Care Plus?
No, not automatically. If you are a HUHC patient, you could be assessed to see whether you are eligible for Care Plus. If yes, then you can be transferred.
