Introduction

Harbour Health covers an extensive geographical area from Devonport, which is part of North Shore City, north to Warkworth in the Rodney District. Practices are located in both urban and rural settings. They cover the spectrum of general practices, ie from a solo practitioner to a large group medical centre that provides accident and medical services, as well as specialist and other allied medical professional services.   

The 36 general practices that are affiliated to Harbour Health are also members of Comprehensive Health Services (CHS).  To see a full list of practices, including contact details, the names of general practitioners at each practice as well as details of additional services they provide, select Affiliated Medical Centres from the menu option, and either scroll through the practice listing or search by location or by GP name.


Harbour Health is governed by a Board of Directors that includes four General Practitioners, as well as Ngati Whatua, and community representatives. Profiles for the Board of Directors can be viewed from the Staff and Board Members menu option. A full list of staff members and a brief outline of their roles are also available from the Staff and Board Members menu option.


Harbour Health has identified critical areas to ensure services and programmes managed and developed by Harbour Health address the health needs of the approx 150,000 enrolled population. These include: 

  • a comprehensive understanding of the population health needs of our community.
  • a robust information management system.
  • development of strong community engagement.
  • a demonstration of health outcomes and the reduction of health disparities.
  • strong leadership and innovation.
  • growth of the capability and capacity of general practice and other providers in the delivery of primary care services.
  • to ensure there are effective clinical and primary care systems to sustain the delivery of primary care services.

To progress these key success areas, Harbour Health is in the process of developing a population health management system which includes health needs analysis and community consultation framework; the development of a population health information system; and we have recently started work on the population health outcomes.

A range of services and programmes are designed by Harbour Health and delivered to the enrolled population through general practices. It is essential that during the development phase of all programmes the needs of the enrolled population are identified and factored into all facets of the programme development. To ensure programmes are appropriately designed  Harbour Health has undertaken both a population Health Needs Analysis (HNA), as well as an analysis of our Demographics. By taking this approach Harbour Health will ensure health needs are met and outcomes measured. For more detailed information of this analysis, see the menu options Demographics and Health Needs Analysis in this section.

Harbour Health has also developed innovative clinical systems which include a systematic approach to Cardiovascular Disease and Diabetes and Respiratory Disease. As a result of these innovations, we will make available IT tools to assist General Practice in the population health data management, screening, risk assessment, care management and patient self management. This system of care also assists with integration between General Practice, NGOs and Allied Health providers in our community.

Another success for Harbour Health has been the Clinical Innovation Award from the Waitemata District Health Board for the ASC Smoke Free programme. This programme has involved working with families to create smoke free homes and to reduce/quit smoking.  We have received significant interest from other DHBs and PHOs about this programme.

Harbour Health has also strengthened the delivery of existing programmes such as Diabetes, Retinal Screening, Respiratory and our Health Promotions and Services to Improve Access.
